Response scale
Each statement is scored from 0 to 4: strongly agree, agree, neutral, disagree, strongly disagree. Disagreement produces a higher risk score because statements describe desirable operating practice.

Dimension formula
Each dimension contains five questions. The answer values are summed, divided by the maximum possible 20 and multiplied by 100.

Overall score and bands
The overall score is the arithmetic mean of the five dimension scores: 0–24 steady, 25–49 watch, 50–74 notable leak and 75–100 severe leak.
